Fatality inquiry reports

The Minister of Justice may order that a Provincial Court judge conduct a public fatality inquiry. The inquiry will determine the identity of the deceased; the date, time and place of death; and the circumstances of death. Once the inquiry is complete the judge provides a written report.

The judge may make recommendations for the prevention of similar incidents, but can't make any findings of legal responsibility.

Report to the Attorney General - Public inquiry into the deaths of Cory Bishop and Ryan Pope 
December 19, 2005

On July 24, 2004 a vehicle operated by Ryan Pope, in which Corey Bishop was a passenger, made a right turn without signalling. Pope failed to pull over for police and a pursuit was initiated. The pursuit entered a residential area where it was terminated by the police. The Pope vehicle continued on at high speed, failed to stop at a stop sign, and was struck by a westbound vehicle. Bishop was pronounced dead at the scene. Pope was pronounced dead at the Royal Alexandra Hospital.

November 16, 2005

On July 31, 1998, 18 year-old Nadia Kanji fell to her death during a parachuting activity supervised by the Calgary Skydive Centre, Inc., at or near their training and jump facility at the Beiseker Airport in Alberta. That facility was also known as the Skydive Ranch, Inc.

November 10, 2005

Stephen Barnard was found unresponsive and hanging by his neck in his cell at the Grande Cache Institution on November 15, 2004.

October 14, 2005

DT was found dead of a drug overdose at the home of his foster parents. He died January 13, 2004.

October 06, 2005

Josif Fekete shot himself after shooting his estranged wife Blagica and their son Alex. They died September 28, 2003.

Report to the Attorney General - Public inquiry into the death of Richard Forbes 
September 21, 2005

Richard Forbes was in an RCMP holding cell when another occupant of the cell struck him on the side of the head. He was transferred to a different cell where he fell a number of times and was found unresponsive. He was seen at the Northern Lights Hospital and then transferred to the University of Alberta Hospital where his condition continued to deteriorate. He died November 18, 2003.

September 20, 2005

AJP was found dead in a vehicle parked in the garage of a private residence. An older male was also found in the vehicle. There was evidence the vehicle had been running and drug paraphernalia was found within it. She was found dead November 20, 2003.

September 06, 2005

Daniel Couterielle was found dead of a self-inflicted gunshot wound after a prolonged standoff with police on July 12, 2003.
