Fatality inquiry reports

The Minister of Justice may order that a Provincial Court judge conduct a public fatality inquiry. The inquiry will determine the identity of the deceased; the date, time and place of death; and the circumstances of death. Once the inquiry is complete the judge provides a written report.

The judge may make recommendations for the prevention of similar incidents, but can't make any findings of legal responsibility.

December 14, 2007

On November 28, 2005, Denise Gillies and Alvin Lockhart were found unresponsive after a fire in the Alberta Hotel in Oyen.

Report to the Attorney General –Public inquiry into the death of Francis McPike 
November 28, 2007

On October 28, 2005, Francis McPike was found unresponsive in this cell. He was transported to hospital where he died on October 31, 2005.

November 23, 2007

On March 26, 2006, Ted James Meiorin became unresponsive shortly after being restrained by police.

November 15, 2007

On August 19, 2005, Joseph Dafoe was found unresponsive and hanging in his cell.

November 08, 2007

On November 23, 2004, Joseph Delorme was taken to the hospital complaining of abdominal pain. He was admitted and died later that day.

October 30, 2007

On June 13, 2005, K.C. was found unresponsive, suspended by his neck, in the basement of his residence. The Child, Youth and Family Enhancement Act prohibits publishing the name of the child.

Report to the Attorney General - Public inquiry into the death of Robert Mervin Moore 
October 30, 2007

On June 25, 2005, Robert Moore fell off a ladder and struck his chest. He went to the hospital the next day complaining of chest pain and had a tube inserted, with instructions to return the next day for follow-up, which he did. On June 28, 2005 he was brought to the hospital complaining of shortness of breath. He experienced a sudden loss of consciousness, which he quickly regained, and was taken to the Observation Unit where he was later found unresponsive.

July 17, 2007

On September 28, 2005, L.S. was being transported to his group home when he attempted to jump from the moving vehicle but was prevented. When the vehicle was stopped he ran onto the highway and was hit by a truck. The Child, Youth and Family Enhancement Act prohibits publishing the name of the child.

Report to the Attorney General - Public inquiry into the death of Charles Jacob Wula 
May 30, 2007

Charles Wula was found drowned in the North Saskatchewan River on July 24, 2005. On July 17, 2005, he was involved in a foot chase with the Edmonton Police Service near Rundle Park. He escaped apprehension by entering the river and was visually followed for about 1 km before officers lost sight of him.

May 22, 2007

On April 21, 2004, Dane Wintermute was the pilot and lone occupant of a Cessna aircraft which crashed near Josephburg, Alberta.

April 04, 2007

Andrew Mazur was discovered having a seizure in his cell on December 26, 2004. He was transferred to the Foothills Hospital where he died on January 8, 2005.

March 16, 2007

On March 12, 2003, A.H. died in the garage of his grandparents' home. The Child, Youth and Family Enhancement Act prohibits publishing the name of the child.

February 14, 2007

Adam Miller and Huu Pham died as a result of injuries sustained after they climbed over the fourth floor balcony railing and fell to the ground during tactical entry by Edmonton Police Service into the apartment where they were located. They died on September 24, 1999.
